Possible Cure for Leukemia Found in Fish Oil

ScienceDaily (Dec. 22, 2011) — A compound produced from fish oil that appears to target leukemia stem cells could lead to a cure for the disease, according to Penn State researchers.

The compound -- delta-12-protaglandin J3, or D12-PGJ3 -- targeted and killed the stem cells of chronic myelogenous leukemia, or CML, in mice, said Sandeep Prabhu, associate professor of immunology and molecular toxicology in the Department of Veterinary and Medical Sciences. The compound is produced from EPA -- Eicosapentaenoic Acid -- an Omega-3 fatty acid found in fish and in fish oil, he said.

"Research in the past on fatty acids has shown the health benefits of fatty acids on cardiovascular system and brain development, particularly in infants, but we have shown that some metabolites of Omega-3 have the ability to selectively kill the leukemia-causing stem cells in mice," said Prabhu. "The important thing is that the mice were completely cured of leukemia with no relapse."

The researchers, who released their findings in the current issue of Blood, said the compound kills cancer-causing stem cells in the mice's spleen and bone marrow. Specifically, it activates a gene -- p53 -- in the leukemia stem cell that programs the cell's own death.

"p53 is a tumor suppressor gene that regulates the response to DNA damage and maintains genomic stability," Prabhu said.

Killing the stem cells in leukemia, a cancer of the white blood cells, is important because stem cells can divide and produce more cancer cells, as well as create more stem cells, Prabhu said.

The current therapy for CML extends the patient's life by keeping the number of leukemia cells low, but the drugs fail to completely cure the disease because they do not target leukemia stem cells, said Robert Paulson, associate professor of veterinary and biomedical sciences, who co-directed this research with Prabhu.

"The patients must take the drugs continuously," said Paulson. "If they stop, the disease relapses because the leukemia stem cells are resistant to the drugs."

Current treatments are unable to kill the leukemia stem cells, Paulson said.

"These stem cells can hide from the treatment, and a small population of stem cells give rise to more leukemia cells," said Paulson. "So, targeting the stem cells is essential if you want to cure leukemia."

During the experiments, the researchers injected each mouse with about 600 nanograms of D12-PGJ3 each day for a week. Tests showed that the mice were completely cured of the disease. The blood count was normal, and the spleen returned to normal size. The disease did not relapse.

In previous experiments, the compound also killed the stem cells of Friend Virus-induced leukemia, an experimental model for human leukemia.

The researchers focused on D12-PGJ3 because it killed the leukemia stem cells, but had the least number of side effects. The researchers currently are working to determine whether the compound can be used to treat the terminal stage of CML, referred to as Blast Crisis. There are currently no drugs available that can treat the disease when it progresses to this stage.

The researchers, who applied for a patent, are also preparing to test the compound in human trials.

I am sitting here at work and kinda in a ba humbug kinda mood and then I read this article and has totally made me put things in perspective- we are lucky. A possible cure is right around the corner. Another good reason for me to keep taking Omega 3 everyday, as I have in the last 2 years. Maybe, just maybe, it has helped my body to react so positively while taking Sprycel.

This is very interesting. Wouldn't it be great if a cure--or even a durable remission--could be accomplished with not-a-drug, but plain old fish oil.

This is hopeful news, it is good to see the research taking place. Keep in mind they are not saying Fish oil does anything for CML, they are saving a new compound that they derived from Fish oil may destroy CML stem cells. Fish oil in general is good for you, but CML patients should not feel they have to go out and start taking Fish oil.

I can understand why Dr. Talpaz may be a bit skeptical...

While it has been reported that Fish oil may kill leukemic stem cells:

We also have to beware that Fish oil may interfere with chemotherapy and we do not yet know if it would interfere with the types of oral TKI's that we take for CML

Bottom line, do not start self medicating with copious amounts of supplements of Fish Oil as it may render the TKI's to become ineffective. BCR ABL needs to be inhibited in order to stop progression of the disease. Be Informed and Be Aware!

This article is not saying that fish oil is not good for health; they are saying that it is not good for particular people facing problem of CML. People are not affecting with this problem can take fish oil supplements after consulting doctors.

I checked with Carolyn Blasdale re taking Fish Oil (thinking it can't hurt...). She felt that there was no issue with taking it together with TKI re inhibiting the effect of the TKI's. She wrote me as follows:

"Fish oil can increase risk of bleeding as it is a platelet inhibitor and dasatinib can also affect platelet function. So, fish oil dose should definitely be less than 3000 mg daily. If your platelet count drops below 100,000 you should stop fish oil."

I don't tend to have an issue with low platelets and have been taking 1000mg of fish oil.

Thank you for posting Carolyn's take on the "Fish Oil" question. As Dr. Druker's nurse, she is a valuable source of CML information.

Did you discuss, with her, the recent research at University Medical Center Utrecht, the Netherlands indicating that "Fish Oil Reduces the Effectiveness of Chemotherapy"? I would very much like to know her and Dr. Druker's thoughts on this research.

Hi Jerry,
I did not discuss the article with her (I kind of assumed that Dr D and Carolyn are on top of the latest when it come to drug interactions).

I have sent her the link to the article, along with the question of whether I should discontinue taking the fish oil. My last PCR, the first after resuming Sprycel, increased from 0.77 to 1.22. Mutations testing came back negative -so maybe there is an effect 9or maybe 2 weeks was not enough time for the Sprycel to get to work)

I will certainly keep you posted.

Hi Jerry et al,
I got the following reply from Carolyn Blasdale:

"The one chemotherapy mentioned, cisplatin, is such a totally different drug form Sprycel or Gleevec that I don't think this applies at all. I will send it on to our pharmacist for his opinion. I have not received any alerts on restricting fish oil with Sprycel except due to its tendency to increase bleeding risk.

On the other hand, I doubt any specific studies have been done on fish oil with Sprycel or Gleevec. If you have any doubt, just stop fish oil as it is optional."

Any thoughts?

Hi to all,
I received the following reply from Carolyn today, re her inquiry to the OHSU pharmacist about whether to take fish oil with TKI's:

"Our expert pharmacist says none of the TKI's were tested as part of this study. The main drugs affected were cisplatin and irinotecan.

It has not been studied with TKI's. such as Sprycel.
The pharmacist says that since fish oil is optional, he would suggest that you stop it until we know more."

So, no more fish oil for me....
